Physiotherapy Clients

Patient Forms

Upon entering our office, our receptionist will welcome you as a member of our family and provide you with the necessary paperwork we will need you to fill out prior to your visit. This paperwork provides us with your health history and information on your condition.

Meet Your Physiotherapist

You will be taken to the second waiting area, where you will meet your physiotherapist and be taken to the treatment room. Your therapist will start by introducing herself, going over the paperwork with you and asking you questions specific to your condition as well as your expectations from physiotherapy.

A thorough assessment would involve asking questions regarding your medical history, method of injury and may also involve moving your body, asking you to perform movements, to walk, neurological testing, strength testing to determine the issue or issues at hand and formulate a diagnosis as well as to know more about you and your concerns. During the assessment, please feel free to ask questions, the more informed you are about the assessment and treatment plan the more effective and efficient your recovery will be.

Once the assessment is completed, your physiotherapist will discuss the findings with you and develop a treatment plan outlining your role and our role in your recovery. Your treatment program would begin with screening for any contraindications and providing information regarding the risks and benefits of the treatment techniques and machines used. The treatment may involve the use of machines (Electrotherapy), heat, cold, exercises and stretches (Exercise therapy) and hands-on-technique.

How Should I Prepare?
  1. Wear non-restrictive clothing, such as cotton shorts, T-shirts and leggings. If the area of your body causing pain is covered by clothing, you may be asked to remove it so your physiotherapist can gain a better understanding of your problem.
  2. Make sure you are well hydrated before and after the treatment.
  3. Avoid application of oils, creams and sunscreens before treatment as they would impede the effectiveness of treatment with machines.
